CHES Directory Form for 2026-2027

Do you need the names of all the kids in the class? Are you trying to set up a playdate or birthday party and don’t know the parents’ names? What was their address again?

The CHES Directory can help! CHES PTA collects and organizes parent/caregiver contact information so that the CHES community can use it appropriately. We put it here on the website on a password-protected page.

Please be sure to submit your directory information each year! (We won’t automatically include kids even if they were in the directory last year.)

You will receive a printed form in your welcome packet for the beginning of the year. Already submitted? You’re all set!
